By submitting my order, I agree to the purchase of the service processing fee(s) described and listed onAPPLY READY DHA Processing Service Q&A and on the company website www.applyready.com. I understand that payment is required to begin processing service. The fee for this service is as stated in APPLY READY’s DHA Processing Service Q&A. This serviceincludes the cost of completing and preparing my DHA application/s and various verification, requests and credential service applications. It does not include the fees charged by the professional regulatory board/s, verification and credential service agencies, legal authorities and testing centers for application and licensure, and various other agencies that charge for third party verification and requests. APPLY READY will provide an itemized list showing third-party institutional fees along with shipping and mailing fees. APPLY READY will begin processing application/s upon receipt of this signed SERVICE AGREEMENT and payment. I understand that this application does not entitle me to a DHA License and that APPLY READY does not issue DHA licenses nor provide legal advice concerning the DHA licensure process. understand that APPLY READY cannot specify processing time since it’s the professional regulatory licensing boards that issue DHA licenses; therefore APPLY READY cannot guarantee time estimates. I agree that APPLY READY and its representatives who provide this information to the professional licensing board/s and various related agencies in good faith shall not be liable for any act or omission on my part related to the evaluation, completion and verification of the information contained in my application. I agree not to hold APPLY READY liable for processing delays caused by incomplete or inaccurate information on my part. Files placed on hold, after a period of inactivity of at least 45 days on the part of the client will later be billed at our company hourly rate to reactivate file or redo any stale documents. By signing this agreement, I acknowledge that I have read and agree with these statements, company policies, services and fees described and listed in APPLY READY’s DHA Processing Service Q&A and on the company website www.applyready.com.

This SERVICE AGREEMENT is issued pursuant to the DHA License Processing Service between DHA Applicant (“Client”) and Apply Ready License Processing Inc (“Service Provider”), effective upon completion of payment (of the “Processing Service”). This SERVICE AGREEMENT is subject to the terms and conditions contained in APPLY READY DHA Processing Service Q&A between the parties and is made a part thereof. Any term not otherwise defined herein shall have the meaning specified in the DHA Processing Service Q & A. In the event of any conflict or inconsistency between the terms of this SERVICE AGREEMENT and the terms of the DHA
Processing Service Q & A, the terms of this SERVICE AGREEMENT shall govern and prevail.

This SERVICE AGREEMENT (hereinafter called the “Agreement”), effective upon completion of payment by the Client, is entered into by and between Service Provider and Client, and is subject to the terms and conditions specified above. Any issue(s) to this AGREEMENT, if any, shall be deemed to be a part hereof. In the event of any
inconsistencies between the terms of the body of this AGREEMENT and any issue(s) hereto, the terms of the body of this AGREEMENT shall prevail.

hereby authorize the following entities and individuals to release all information in their

possession concerning me, whether oral, in writing, documented or other, to APPLY READY LICENSE PROCESSING INC. and/or its agents acting on my behalf.

All professional licensure boards, verification and credential service agencies, legal background agencies and testing authorities.

All schools or universities which I have attended.

All institutions at which I have ever received training and all facilities at which I have ever held professional work privileges, whether full or limited, temporary or permanent.

All professional institutions, specialty boards, and all other organizations with which I have ever been associated.

All attorneys who have ever participated in criminal or civil actions, in which I was named party, that would pertain to or directly affect my ability to obtain DHA License, practice my profession and/or have professional privileges.

I hereby make this release and waiver of rights for the purpose of allowing APPLY READY LICENSE PROCESSING INC. and its agents to execute its duties pursuant to my request for DHA Application processing service.

I further authorize APPLY READY LICENSE PROCESSING INC. or any of its duly authorized agents to submit verification requests, professional inquiries and applications relevant to the requirements of DHA licensure.

A photocopy of this Release, Authorization and Waiver of Rights shall be as effective as the original.
